The National Transportation Safety Board investigated the 2018 crash on the 405 Freeway. They determined that a driver eating a bagel and drinking coffee was not paying enough attention when his Tesla slammed into a parked fire truck. The NTSB also reports that the car’s autopilot system failed to detect the fire truck when a different car suddenly pulled out of the way. No one was injured in the crash.
